An honest comparison from Cima EHR

Kipu alternative for outpatient programs: an honest comparison

Kipu Health sells an EMR, an admissions CRM with a bed board, revenue cycle software and a compliance product to substance use, behavioral health, eating disorder and medication-assisted treatment programs. It doesn't publish prices; you book a consultation. If you run detox or residential beds, that breadth matters and Kipu is the stronger fit. If you run only PHP, IOP and outpatient for up to 25 clients, Cima EHR's Program plan does the outpatient program day for a published $399 a month, month to month. Below: what each covers, the contract terms to read, a decision guide, and what Cima doesn't do.

Updated October 24, 2026. Kipu facts come from kipuhealth.com, read on that date. Written by the team that runs a licensed Texas PHP, IOP and outpatient program and built Cima EHR.

What Kipu is built for

Kipu describes itself as software for the whole treatment journey. Its products page lists four main products: a customer relationship management system for admissions, an electronic medical record, revenue cycle management, and governance, risk and compliance. Its who we support page names substance use treatment centers, behavioral health treatment centers, eating disorder centers and medication-assisted treatment programs.

  • EMR with medication workflows. Kipu's EMR page describes medication orders and administration, documenting medications patients bring in, patient and facility medication inventories, and controls against diversion of drugs such as buprenorphine and methadone. Those are residential and detox needs an outpatient-only EHR usually doesn't cover.
  • Admissions CRM with a bed board. Its CRM page covers intake from pre-admission on, with bed availability shown from the EMR.
  • Revenue cycle. Its RCM page lists eligibility checks, utilization review, claim creation and submission, payment posting, patient statements, follow-up and reporting.
  • E-prescribing. Kipu publishes Surescripts electronic prescription service terms, so e-prescribing runs through the Surescripts network.

That's a broad platform for multi-level organizations. The question for an outpatient-only program is whether you need the residential half of it.

Kipu vs Cima, criterion by criterion

"Ask Kipu" isn't a knock. Program EHRs often show these things in a demo rather than on a web page. Run the eight-step program-day demo with any vendor, us included, and score what you see.

Contract terms to read, whoever you choose

Kipu publishes its general terms of service, which is more than many vendors do. A few clauses there are worth knowing before a quote, because they're common across enterprise EHR contracts and change the real price:

  • Yearly increases. The terms reserve the right to raise subscription fees once a year by the greater of the CPI increase or 5%, starting no earlier than the first anniversary of the order. Over three years, a 5% yearly increase adds about 10% by year three.
  • Charges outside the agreement. Goods, services or features you use that aren't in your agreement are billed at then-current rates; the terms name lab, HL7 and API fees as examples. Ask which of your needs are in the base quote.
  • Payment timing. Invoices are due within 15 days unless the order form says otherwise, with a late fee or suspension if unpaid.

Your signed order form controls, and it can differ from the general terms. Ask any vendor for the same things in writing: the term and renewal date, the yearly increase cap, what an export costs when you leave, and how long records stay readable after you cancel. Our guide to EHR contract terms that change the price has the full list. Have your attorney read anything multi-year; Cima isn't a law firm.

A decision guide by program type

What Cima costs a typical outpatient program

An example, not an average: a program with four clinicians, about 20 clients, sending 300 per diem and session claims a month. On Cima that's the $399 Program plan, three added clinicians at $39 each, and 300 claims with their ERAs at 30¢ + 20¢: $666.00 a month, with no setup fee, no add-ons, AI note drafting included, and billing and front-desk logins free. Ask Kipu, and every other vendor, for the same scenario as one monthly number. The Cima price list and estimator shows every line.

What Cima doesn't do

If one of these is a must-have, Cima isn't your EHR today, and we'd rather say so now:

  • No detox, residential or recovery-housing levels; no bed board.
  • No medication administration record, lab orders or results.
  • E-prescribing isn't live yet.
  • No admissions CRM or referral pipeline.
  • No built-in video (you add your own HIPAA-ready video link; groups can be marked as video).
  • The Columbia suicide screen (C-SSRS) isn't built in; PHQ-9 and GAD-7 are, with an immediate alert on the self-harm question.
  • Over 25 clients in the program, talk to us first.

If you move: a program cutover

  1. Check your current contract's renewal date and notice period, and request your full export in writing before you give notice.
  2. Set up Cima's Program settings: each level's day and week hours and per diem code, your claim form (837P or 837I with revenue codes and type of bill), groups and facilitators. Our IOP and PHP billing codes guide has the codes.
  3. Import the client list from a CSV, then enter each current client's level of care, authorization and treatment plan before their first group in Cima.
  4. Enroll for ERAs with each payer (1 to 3 weeks each).
  5. Cut over on a Monday. From that day, groups and per diem days are documented and billed in Cima; finish older claims in the old system until paid. More in switching EHRs.

Questions

How much does Kipu cost?

Kipu doesn't publish prices on its website; it quotes after a consultation. Its public terms of service describe subscription fees set in an order form, a possible yearly increase of the greater of CPI or 5% after the first year, and separate charges for things outside the agreement such as lab, HL7 or API fees. Ask for every line in writing as a monthly total for your program.

Is there a cheaper EHR than Kipu for an IOP?

For an outpatient-only program, there are EHRs with published prices. Cima's Program plan is $399 a month for up to 25 clients plus $39 per added clinician, month to month. Whether it's cheaper than your Kipu quote depends on that quote, and on whether you need residential features Cima doesn't have.

Does Cima work for detox or residential?

No. Cima's levels of care are PHP, IOP and outpatient, and it has no medication administration record or bed board. If you run beds, choose a full-continuum EHR.

Can we keep Kipu for residential and use Cima for outpatient?

You can, but two EHRs mean two charts for a client who steps down, and records shared between them under your consents. Most organizations are better off with one system that covers every level they run.

Does Quick VOB work with Kipu?

Yes. Quick VOB works on its own next to Kipu or any other EHR: verify benefits from a card photo and attach the PDF to the chart.
