An honest comparison from Cima EHR
Alleva alternative for outpatient programs: an honest comparison
Alleva is a behavioral-health-only EMR for detox, residential, PHP, IOP and outpatient programs, with an admissions CRM, built-in billing, an ambient AI scribe (Echo), a client and family portal and a compliance product (InCheck). It has announced ONC certification, and it quotes prices after a demo. If you run beds, are a CCBHC, or want ambient AI and compliance software in the same platform as the chart, Alleva is the stronger fit. If you run only PHP, IOP and outpatient for up to 25 clients, Cima EHR's Program plan covers the outpatient program day for a published $399 a month, month to month. Below: a side-by-side table, how the AI, insurance checks and compliance pieces really differ, who each fits, and how a switch works.
Updated October 24, 2026. Alleva facts come from helloalleva.com, its help center and its press releases, read in October 2026 and listed under Sources. Written by the team that runs a licensed Texas PHP, IOP and outpatient program and built Cima EHR.
What Alleva is built for
Alleva says on its home page that it works only in behavioral health, for programs from startups to multi-location organizations, and names intensive outpatient and outpatient, partial hospitalization and residential programs; its guides add detox. It groups its products into four families:
- Alleva Intelligence: Insights (analytics), Echo (an ambient AI scribe) and Travis (an AI assistant), described on its Alleva Intelligence page and in a July 2026 release.
- Alleva Core: the EMR with billing and medication management. Its addiction treatment EMR guide describes ASAM-aligned treatment plans that carry across levels of care, group notes for IOP and PHP, and e-prescribing with drug-interaction alerts. A release introduced its integrated billing.
- Alleva Connect: a client and family portal with telehealth.
- Alleva Compliance: InCheck, a governance, risk and compliance system for audits, standards, policies, corrective actions and life-safety work, aimed at CARF, Joint Commission and state requirements. Alleva announced its official launch in February 2026.
Alleva also announced ONC certification and SOC 2 for its platform. Cima EHR isn't ONC-certified. Most private PHP/IOP/OP programs billing commercial payers don't need certification, but some buyers do: SAMHSA's CCBHC certification criteria (criterion 3.b.3) call for technology certified under the ONC Health IT Certification Program. If you are or will be a CCBHC, that alone settles it in Alleva's favor over Cima.
Alleva vs Cima, criterion by criterion
Alleva column: the pages linked on this page, read in October 2026. Some of it comes from Alleva's blog guides rather than product documentation, so confirm it in a demo. "Ask Alleva" means we didn't find it published. Rows follow our IOP and PHP EHR buyer's checklist; the four-vendor IOP EHR comparison has the same rows for other vendors.
| Criterion | Alleva | Cima EHR (Program plan) |
|---|---|---|
| Levels of care | Detox, residential, PHP, IOP and outpatient | PHP, IOP and OP only, each with its own day and week hours and per diem code; step-down on the same chart |
| Admissions | CRM; self-service insurance verification in the EMR | No CRM. Eligibility checks on the chart; our separate Quick VOB for admissions |
| Group notes | Group documentation for IOP and PHP described in its guides; ask to see each client's own entry | Topic and summary once, each client's own entry tied to a plan goal, one PIN signs all; entries 80% alike are stopped |
| Attendance and per diem | Its PHP guide describes tracking group and individual sessions; ask how billable hours become a per diem claim | Hours from signed notes; one per diem charge per day that meets its hours; short days flagged, not billed |
| Claims | Integrated billing: charge capture, claims, insurance verification, patient payments; ask which claim forms | 837P, or 837I with your revenue code per level and type of bill; claims and ERAs at 30¢ and 20¢ |
| Utilization review | Ask to see a continued-stay review | Authorizations on the chart; continued-stay review on Today 5 days before the last authorized day, ASAM dimensions 1 to 6 required |
| Medications | Medication management and e-prescribing with interaction alerts | Medication list, allergies, single doses given in the office; e-prescribing not live yet; no scheduled med pass |
| AI notes | Echo, an ambient scribe that listens to the session; Alleva says recordings aren't kept | Drafts from a few lines you type or dictate, on AWS under a BAA; included |
| Portal and video | Client and family portal with telehealth | Paperwork and booking by text link; no built-in video |
| Compliance | InCheck inside the platform | Separate product, Comply, with no client data in it |
| Certification | ONC certification and SOC 2 announced | Not ONC-certified; BAA with every account |
| Price | Not published; demo-led quote. Help center lists insurance checks at $0.60 each | $399 a month for up to 25 clients, $39 per added clinician, no setup fee |
| Term and leaving | Set in your agreement; ask for export terms in writing | Month to month; full export any time; readable at least 90 days after cancelling |
AI notes: ambient scribe vs drafting from your words
Both EHRs use AI on notes, in different ways, and the difference matters for consent and for what ends up in the chart.
- Alleva's Echo is an ambient scribe: it listens to an in-person or telehealth session and produces a structured note. Alleva's guide to HIPAA-compliant AI therapy notes describes a zero-retention policy for the audio. The upside is less typing. The work it adds is a consent process for recording, and in an SUD program a clear answer on how session audio is handled under 42 CFR Part 2. Ask to read the BAA language on audio.
- Cima's drafting starts from a few lines the clinician types or dictates after the session and drafts the note's sections for them to edit and sign. Nothing listens to the session. It runs on AWS's HIPAA-eligible AI service under a Business Associate Agreement, the owner turns it on, and it's included in every plan.
Either way, the clinician signs the note and owns what it says. In a group, payers look for each client's own participation and progress toward their plan goal; our guide to group therapy notes for an IOP shows what that looks like.
Insurance checks: what each costs per check
Admissions run on verifications. Alleva added a self-service verification of benefits tool in 2025 (FinTech Global coverage), and its help center lists the cost as a standard $0.60 per check, billed monthly, run through Waystar. Cima runs eligibility checks from the chart at 30¢ each, at the clearinghouse's rate. For admissions teams who want the deductible, out-of-pocket and prior-auth answer from a card photo, next to any EHR, there's Quick VOB. Whatever you use, an electronic 271 doesn't answer everything for PHP and IOP; the VOB checklist lists what still takes a call.
Compliance software: in the EHR or beside it
Alleva puts compliance (InCheck) in the same platform as the chart. Cima keeps it separate: Comply schedules fire drills, environment-of-care rounds and chart reviews with photos and fixes, and builds the survey binder for Joint Commission or CARF, with its own sign-in and no client records in it. One platform means one login and one vendor; a separate tool means a surveyor-facing system that holds no PHI and can be used with any EHR. Both are reasonable. Ask Alleva which accreditation standards InCheck maps and whether it's priced separately. For the survey itself, see the Joint Commission behavioral health checklist or CARF survey prep.
Who each fits
| If you run… | Lean toward | Why |
|---|---|---|
| Detox or residential, with or without outpatient | Alleva or another full-continuum EHR | Beds, medication management and residential levels; Cima has none of them. |
| A CCBHC, or plan to become one | Alleva or another ONC-certified EHR | SAMHSA's criteria call for certified health IT; Cima isn't certified. |
| Telehealth groups and a family program online | Alleva | Built-in telehealth and a family portal; Cima has no built-in video. |
| One PHP/IOP/OP site, up to 25 in the program | Cima, after you've scored both | The outpatient program day (groups, hours, per diem, UR) at a published monthly price with no term. |
| A new outpatient program, not open yet | Cima, or either after a demo | Low fixed cost while census ramps; Cima Consulting's IOP / PHP Program Launch includes six months of Cima EHR. |
What Cima costs, as an example rather than an average: four clinicians, about 20 clients and 300 claims a month is $666.00 a month on the Program plan, claims and ERAs included, AI drafting included, billing and front-desk logins free. Ask Alleva to quote the same month, including setup, e-prescribing, Echo, InCheck and verification fees. See every line on Cima's pricing page and the features on Cima for IOP and PHP; our EHR pricing guide explains how to compare a quote with a price list.
What Cima doesn't do
- No detox, residential or recovery-housing levels; no bed board or admissions CRM.
- No scheduled medication pass; e-prescribing and lab orders aren't live yet.
- No built-in video or family portal; no ambient scribe.
- Not ONC-certified.
- The Columbia suicide screen (C-SSRS) isn't built in; PHQ-9 and GAD-7 are, with an immediate alert on the self-harm question.
- Over 25 clients in the program, talk to us first.
If you move: a program cutover
- Check your agreement's renewal date and notice period, and ask Alleva in writing for a full export: client list as CSV, signed notes and documents as PDFs, claims and payments, and how long you can read the old system after the end date. Alleva's own guides advise buyers to get export formats and fees in writing; hold every vendor to that, us included.
- Import the client list into Cima from the CSV. The preview flags rows to fix; nothing is saved until you press Import.
- Set up the Program settings: levels with day and week hours and per diem codes, 837P or 837I with revenue codes and type of bill, groups and facilitators (codes in the IOP and PHP billing codes guide).
- For current clients, enter level of care, open authorization and treatment plan, and upload the latest plan, intake and releases to the Files tab.
- Enroll for ERAs with each payer, 1 to 3 weeks each.
- Cut over on a Monday, work old claims to payment in Alleva, and keep its export for your retention period (Texas retention rules). The full plan is in switching EHRs.

Questions
How much does Alleva cost?
Alleva doesn't publish subscription prices; it quotes after a demo. Its help center lists insurance verifications at $0.60 each, billed monthly. Ask for every other line at your census as one monthly number, including setup, e-prescribing, AI, InCheck and any billing service fee.
What is Alleva EMR?
A behavioral-health-only EMR for detox, residential, PHP, IOP and outpatient programs, with a CRM, integrated billing, the Echo ambient AI scribe, a client and family portal and the InCheck compliance product. It has announced ONC certification and SOC 2.
Is Cima cheaper than Alleva for an IOP?
Cima's Program plan is $399 a month for up to 25 clients plus $39 per added clinician, month to month. Whether it's cheaper depends on your Alleva quote and on whether you need beds, video, an ambient scribe or ONC certification, which Cima doesn't have.
Does Cima have an ambient AI scribe like Echo?
No. Cima's AI drafts the note from a few lines the clinician types or dictates after the session; nothing records the session. The clinician edits and signs.
Does Cima work for detox or residential?
No. Cima covers PHP, IOP and outpatient only, with no bed board or medication pass.
Can we keep Alleva for residential and use Cima for outpatient?
You can, but a client who steps down then has two charts, with records shared between them under your consents. Most organizations do better with one EHR that covers every level they run.
