S9480: intensive outpatient psychiatric services, per diem
The short answer: S9480 is the HCPCS code for a mental health intensive outpatient program (IOP) day: "intensive outpatient psychiatric services, per diem." It's billed as one unit per qualifying day, usually with revenue code 0905 on a UB-04. It's an S code, a temporary national code created for private payers, so Medicare doesn't pay it. Commercial plans and TRICARE do, and so do some Medicaid plans, including Texas Medicaid plans offering adult IOP. Because the descriptor names no hours, no services and no diagnosis, your contract fills in all three. Below: what that means, how to set it up, S9480 vs H0015 and 90853, and the documentation behind each day.

The S9480 description
Intensive outpatient psychiatric services, per diem
HCPCS Level II descriptor from the CMS code set (CMS HCPCS quarterly update files; also at HCPCS.codes, coverage code I, not payable by Medicare).
Six words, and each one matters:
- Intensive outpatient: more than weekly therapy, less than partial hospitalization. A client who needs about 20 hours a week belongs in PHP (H0035 or S0201), not S9480.
- Psychiatric: the program treats mental health conditions. A substance use IOP bills H0015; a co-occurring program bills by the track the client is in and the primary diagnosis.
- Services: plural and unnamed. Groups, individual sessions, family sessions and psychiatric check-ins inside the day are all part of it unless the contract carves one out.
- Per diem: one unit is one day. Unlike H0015, whose descriptor sets 3 hours and 3 days a week but no unit, S9480 sets the unit and leaves the hours to the payer.
What an S code means for you
HCPCS Level II S codes are temporary national codes that private payers asked for, for services Medicare doesn't cover or doesn't have a code for. Three practical consequences:
- Medicare never pays it. The CMS file lists S9480 with coverage code I, not payable by Medicare, and there's no Medicare fee schedule amount or RVU to anchor a rate to. Your rate is whatever the contract says.
- Medicaid acceptance varies by state and plan. You'll read that "Medicaid doesn't use S codes." That isn't true everywhere: Texas Medicaid plans list S9480 for mental health IOP (below), Arizona's Medicaid manual sets same-day rules for it, and Nevada Medicaid opened S9480 for billing by certain provider types in 2026. Others use H codes or revenue codes alone. Ask each plan.
- "Temporary" doesn't mean going away soon. S codes can stay in the code set for years. Check the current CMS file each January for changes rather than assuming.
What your contract has to define
Because the descriptor is so thin, an S9480 contract is where the rules live. Before you sign, or before your first claim under an existing contract, get these answers in writing and put them in your billing policy:
| Question | Why it matters | Examples we've seen published |
|---|---|---|
| How many hours make a day? | The per diem is paid only for a qualifying day; a short day isn't billable. | New Mexico pays its S9480 daily rate for at least 3 hours of service; many commercial contracts say 3. |
| Which services count toward the hours? | Some plans count groups and individual therapy but not a medication visit or a screening. | Written into the contract's service definition or the plan's medical policy. |
| What can't be billed the same day? | Same-day edits deny the second claim. | Arizona Medicaid's manual says other day-treatment, IOP, PHP and inpatient per diem codes may not be billed on the same day as S9480; one plan's policy calls the per diem bundled and won't pay individual services billed with it. |
| Which form and revenue code? | A UB-04 without the right revenue code, or a CMS-1500 when the plan wants a UB-04, rejects. | 0905 is the usual pairing; TRICARE accepts 0905 or 0906 with S9480. |
| Which provider types can bill it? | A mental health IOP's license and enrollment have to match what the plan contracts for. | Ask in writing; in Texas, see licensing a mental health IOP or PHP. |
| Is virtual IOP covered, and how is it billed? | The code stays S9480; the modifier and POS change. | See running a virtual IOP. |
| Prior authorization and review schedule? | Days are authorized in blocks; a lapsed authorization is the most common lost day. | The prior authorization guide walks through it. |
The thresholds payers publish for IOP days generally, and what counts as an hour, are on per diem billing for IOP and PHP.
Diagnosis and medical necessity
S9480 doesn't name a diagnosis, but "psychiatric" does the work: the primary diagnosis on the claim should be the mental health condition the program treats (depression, bipolar disorder, an anxiety or trauma-related disorder and so on, from the F-chapter of ICD-10-CM outside the F10–F19 substance use block; see the CDC's ICD-10-CM files). Two rules that keep it clean:
- Co-occurring clients: list the condition the client is in this program for first. A client with major depression and an alcohol use disorder in your mental health track: S9480, depression first, the F10 code secondary. If that client later moves to your substance use track, the code changes to H0015 from the date of the change, with a documented level-of-care decision.
- Medical necessity isn't ASAM. The ASAM Criteria are for substance use. For mental health IOP, plans use their own criteria or a licensed guideline set (we compare them in IOP vs. PHP vs. outpatient). Ask each plan which one it uses and write your admission, continued-stay and discharge criteria to it. The habits in continued-stay documentation still apply: what changed, why a lower level won't do, and when the client steps down.
Revenue code, place of service and modifiers
- Revenue code (UB-04): 0905, intensive outpatient services, psychiatric, is the usual partner. Superior HealthPlan's Texas Medicaid mapping is S9480 with 0905, with the HCPCS code required and the revenue code optional (Superior, September 2025). TRICARE accepts 0905 or 0906 with S9480.
- Place of service (CMS-1500): the payer decides; 11 (office) is common for a freestanding program, and 53 applies only if you're a community mental health center, from the CMS place of service code set. Use 02 or 10 for telehealth where covered. A UB-04 has no place of service.
- Modifiers: often none. Some Medicaid programs want HE (mental health program); telehealth may need 95, 93 or GT. Use only what the payer's guide lists; see the modifier table.
Whether a contract wants a UB-04 or a CMS-1500 at all is covered in UB-04 vs CMS-1500.
S9480 and Medicare
"S9480 Medicare," "S9480 CMS," "is S9480 covered by Medicare": the answer to all three is that Medicare doesn't pay S9480 to anyone. Medicare does cover mental health IOP, since January 1, 2024, but differently:
| Commercial S9480 | Medicare IOP | |
|---|---|---|
| Who can bill | Any program the plan contracts with | Hospital outpatient departments, community mental health centers, FQHCs and rural health clinics only |
| How the day is coded | One S9480 line per day | Each service on its own line (psychotherapy, group psychotherapy in a PHP or IOP setting such as G0410, and so on), on a UB-04 with condition code 92 (CMS MM13222) |
| How it's paid | The contract's per diem | An OPPS per diem for days with 3 services or 4 or more |
| Hours | The contract's definition | A physician certifies the need for at least 9 hours a week |
A freestanding mental health IOP isn't one of Medicare's provider types, so it can't bill Medicare for the day; its enrolled clinicians can bill their own services. For dual-eligible clients, Medicare is primary, so verify before admission. The details, including the CMHC route, are in Medicare coverage of IOP and PHP.
Who pays S9480
| Payer | S9480? | What to know |
|---|---|---|
| Commercial plans | Yes, the usual mental health IOP code | Prior authorization, one unit per day, the contract's day definition and same-day rules. The behavioral health benefit may be managed by another company (carve-outs). |
| Texas Medicaid (managed care) | Yes, through plans that offer adult IOP | Adult IOP is an in-lieu-of service each plan may offer (HHSC Uniform Managed Care Manual 16.3); under 21, through Texas Health Steps. HCPCS required on IOP claims since July 1, 2025 (Superior). See Texas Medicaid for IOP and PHP programs and how to look codes up at TMHP. |
| Medicare | No | See above. |
| TRICARE | Yes | UB-04 with 0905 or 0906, one line per date; accredited, TRICARE-authorized programs only. |
| Other states' Medicaid | Some | Nevada, Arizona and New Mexico publish S9480 rules; others use H codes. Read the state manual. |

S9480 vs H0015, 90853 and its neighbors
| Code | What it is | Unit | How it relates to S9480 |
|---|---|---|---|
| H0015 | Alcohol and/or drug intensive outpatient, at least 3 hours a day, 3 days a week | Usually a day | The substance use IOP code. Same level, different program and primary diagnosis. Don't bill both for one client on one day. |
| 90853 | CPT group psychotherapy (other than a multiple-family group) | Session | One group session. Inside an S9480 day, groups are already paid. Use 90853 for outpatient clients, or where a contract pays groups separately. |
| S0201 / H0035 | Partial hospitalization, less than 24 hours | Day | The level above IOP. A step-down from PHP to S9480 changes the code on the date of the change. |
| S9484 / S9485 | Crisis intervention mental health services, per hour / per diem | Hour / day | Crisis services, not an IOP. Easy to mistype; a payer that sees S9485 on an IOP claim will deny or misprice it. |
| H0035 vs S9480 | PHP vs IOP | Day | Bill the level the authorization covers on that date, never the higher one because the day ran long. |
S9480 documentation requirements
Payers' lists differ in wording; these are what every one we've worked with checks for each billed day:
- A psychiatric diagnostic evaluation with the diagnosis and the reason the client needs IOP now, against the plan's criteria.
- A current individualized treatment plan, signed, reviewed on the plan's schedule, with goals the day's notes name.
- A signed note per session with start and end times and the client's own participation and response, not a copied group summary (group therapy notes for IOP).
- Hours that meet the contract for the date, from signed notes alone.
- Safety: screening scores (PHQ-9, GAD-7, a suicide risk screen) over time, and a safety plan when risk is present.
- An authorization covering the date and level, and continued-stay reviews for each new block.
- Psychiatric oversight the contract requires, such as a prescriber's evaluation or periodic review, documented in the chart.
The chart audit checklist reads charts the way a payer auditor does, and the billing checklist covers the claim side.
S9480 in Cima
In Cima's Program plan for IOP and PHP, the IOP level defaults to H0015; a mental health program switches it to S9480 under Settings, Program, sets the hours its contracts require (default 3 a day, 9 a week), and adds up to four modifiers such as HE. Each client-day becomes one unit of S9480 only when signed notes reach the day's hours. Facility claims carry a revenue code per level, which you set to 0905 for a mental health IOP. Individual sessions on a program day are part of the per diem by default; if a contract pays them separately, turn that on and they stop counting toward the day's hours, so the same hour isn't paid twice. Clients fill in the PHQ-9 and GAD-7 on their phone, and any answer above "Not at all" on the PHQ-9 self-harm question puts a safety card on the clinician's Today screen. One honest limit: the code is set per level, not per payer, so a program that bills S9480 to one plan and H0015 to another for the same level should talk to us before go-live.

A mental health IOP, billed from the notes
Cima's Program plan is $399 a month for up to 25 clients: levels of care, group notes with each client's own entry, screenings, attendance hours, S9480 or H0015 per diem claims, and authorizations. Each added clinician is $39 a month.

Questions
What is S9480?
The HCPCS Level II code for intensive outpatient psychiatric services, per diem: one day of a mental health IOP, billed as one unit. It's an S code, a temporary national code for private payers.
Is S9480 a CPT code?
No. It's a HCPCS Level II code from CMS's code set. CPT codes, like 90853, are the AMA's Level I codes.
Does Medicare cover S9480?
No. S codes aren't payable by Medicare. Medicare covers mental health IOP only from hospitals, CMHCs, FQHCs and rural health clinics, billed service by service with condition code 92.
What revenue code goes with S9480?
Usually 0905, intensive outpatient services, psychiatric. TRICARE accepts 0905 or 0906. Superior HealthPlan's Texas Medicaid mapping is S9480 with 0905.
How many hours is an S9480 day?
The descriptor doesn't say. The payer or contract sets it, commonly at least 3 hours. New Mexico Medicaid, for example, pays the S9480 daily rate for at least 3 hours of service.
What diagnosis code goes with S9480?
A mental health condition as the primary diagnosis, outside the F10–F19 substance use block. For co-occurring clients, list first the condition the client is in this program to treat.
What modifiers go with S9480?
Often none. Some Medicaid programs require HE for a mental health program, and telehealth may need 95, 93 or GT. Use only modifiers the payer lists.
Does Texas Medicaid pay S9480?
Through managed care plans that offer adult IOP as an in-lieu-of service, and through Texas Health Steps for members under 21. Superior lists S9480 with revenue code 0905 and has required the HCPCS code since July 1, 2025. Ask each plan.
What is the difference between S9480 and H0015?
S9480 is a mental health IOP day; H0015 is a substance use IOP day of at least 3 hours, 3 days a week. Match the code to the program the client is in and the primary diagnosis.
What is the difference between S9480 and 90853?
S9480 is a whole IOP day; 90853 is one group psychotherapy session. Groups inside an S9480 day aren't billed separately unless the contract says so.
Does S9480 have RVUs?
No Medicare RVUs or fee schedule amount, because Medicare doesn't pay it. The rate is in each payer's contract.
